Adducts deserve a paragraph. Sodium and potassium are everywhere in glassware and buffers, they shift the observed mass by amounts that look meaningful, and an analyst who is not expecting them can report a modification that is really a salt. It is the most common false positive I see in this trade.

The monoisotopic against average distinction is the one I spend most time explaining. Below about two kilodaltons the monoisotopic peak is the one you can see and the one you should quote. Above it the isotope envelope broadens until the average is the honest description, and a certificate quoting a monoisotopic mass for a large peptide is quoting a peak nobody resolved.

Which is why we ask which mass a document is reporting before we compare it with anything. The two figures differ by more than a dalton on a molecule of this size, and the difference has been read as an impurity more than once.

Any transposition of two residues preserves the intact mass exactly. So does a substitution that swaps one residue for another of the same composition. The set of molecules consistent with a given mass is not small, and describing a mass match as identity confirmation makes a claim about that whole set.

The claim that a reproduced spectrum is worth more than any number in the document seems overstated. Most buyers cannot read a spectrum, and a printed image invites false confidence rather than scrutiny.

Partly conceded. A spectrum is worth more to a reader who can read one, and this department exists partly to increase that number. But it is also an artefact that can be checked by a third party later, which a bare verdict is not, and that alone justifies printing it.

You might state plainly that absorbance at the peptide bond is not uniform across sequences either. Two impurities present at the same molar concentration will give different areas if one carries an extra tryptophan, and at 214 nanometres the backbone dominates but does not monopolise. Area per cent is an approximation twice over.

The wavelength section is the part of this series I have found most useful in practice. A supplier sent us a certificate reading at 280 nanometres for a sequence with a single tyrosine, and once we asked for 214 nanometres the impurity profile looked entirely different. Nobody had done anything improper. The method simply could not see what we needed to see.

Gallbladder-related findings follow rapid weight loss from any cause and are not specific to any pharmacology. Attributing them to the agent rather than to the rate of loss is a common error and the distinction matters for what somebody does next. This is not medical advice.

Panel names imply organ specificity that the analytes do not have. Several of the markers in a liver panel are produced elsewhere and rise for reasons unconnected with that organ, which is why the panel is read as a pattern rather than item by item.

Biological variation within one person is smaller than the variation between people for many analytes, which is why a series of results on one individual is far more informative than a single result against a population interval.

You give the reference change value for ALT as about sixty per cent, which strikes me as so large as to make routine monitoring of it pointless. Is that your position?

Not quite. It makes monitoring for small movements pointless, which is different. A doubling is well outside the RCV and is a real signal; a rise from 28 to 41 is not. The value of the test lies in detecting the former, and much of the anxiety it generates comes from acting on the latter.

Intervals are periodically revised, and a laboratory that changes its interval will produce an apparent change in a patient whose biology did not move. The revision date is on the report and nobody reads it.

An accreditation body will investigate a complaint about a laboratory within its scope. Almost nobody in this market knows that, and I would put the route in a sidebar: it is free, it is real, and it is used vanishingly rarely.

What happens when a certificate turns out to be wrong is the question your article stops short of. In a regulated setting a corrected document is reissued to everyone who received the original. Elsewhere it is corrected for whoever complained, and the original stays in circulation. That asymmetry is worth a piece of its own.

A certificate for a molecule is not a certificate for a vial. Where a supplier publishes one document per product rather than one per lot, the document is marketing with a chromatogram attached, however genuine the underlying determination was.

Reconstituting with the wrong volume and then calculating from the intended volume is the second error, and it is invisible afterwards because the vial looks identical. Writing the actual diluent volume on the vial with a marker takes three seconds and closes it.

The convenient reconstitution is the one that makes the arithmetic trivial rather than the one that makes the volume small. Choosing a diluent volume so that a graduation corresponds to a round figure removes an entire class of error, and it costs nothing but a moment’s thought before the needle goes in.

As a practice nurse I would add the ten-second hold to your list of things people skip. I watch patients withdraw immediately and then wonder about the wet patch on their skin. It is the most visible underdose there is and almost nobody connects the two.

Freight is the confounder nobody controls for. Two vials from the same lot, one shipped in January and one in July, are not the same sample by the time they reach an instrument, and a blind programme that ignores the season is measuring the courier as much as the manufacturer.
